Medical Cannabis Program Enrollment

If you are interested in enrolling in the New Mexico Medical Cannabis Program, we are here to help with every step of the way.
From medical record retrieval and review, to scheduling with our providers, to application assistance and preparation, we can do it all.

All patients will need a valid New Mexico ID. As of October 2019, patients with a qualifying diagnosis
and a valid out-of-state driver's license may apply to the NM Medical Cannabis Program. 
Insurance currently does not cover the program.
We are located in Albuquerque, New Mexico.

How To Get Started:

Patients diagnosed with any approved condition for enrollment may begin by submitting current, thorough, printed medical records to our office for review with our provider, or by signing a release of information with our office so that we may request medical records.
Once medical records have been reviewed and accepted by our provider, patients may move forward with scheduling an appointment towards enrollment in the New Mexico Medical Cannabis Program. The total for new patients with qualifying medical records is $95 + tax. We offer veteran and 65+ discounts.
Standard, VA, and UNMH releases of information are available at Zia Health & Wellness.

P.T.S.D. 

For patients with a current Post-Traumatic Stress Disorder diagnosis, current medical records may be submitted to our office for our provider's review towards enrollment. With acceptable records, the total for enrollment is $95 + tax.
For patients who do not already have a current diagnosis of Post-Traumatic Stress Disorder, we have providers on site who can evaluate patients for P.T.S.D. towards enrollment in the New Mexico Medical Cannabis Program. The total for an evaluation is $140 + tax. We offer veteran and 65+ discounts. Patients are offered a full refund if unable to be diagnosed towards enrollment under P.T.S.D.

Opioid Use Disorder

For patients with a current Opioid Use Disorder diagnosis, current medical records may be submitted to our office for our provider's review towards enrollment. With acceptable records, the total for enrollment is $95 + tax.
For patients who do not already have a current diagnosis of Opioid Use Disorder, we have providers on site who can evaluate patients towards enrollment in the New Mexico Medical Cannabis Program. The total for an evaluation is $140 + tax. We offer veteran and 65+ discounts. Patients are offered a full refund if unable to be diagnosed towards enrollment under Opioid Use Disorder.

Card Renewals

Patients who need to renew their New Mexico medical cannabis card may give us a call to schedule a renewal at (505) 299-7873.
Renewing patients will need their expiring medical marijuana card and valid New Mexico ID. We offer veteran and 65+ discounts, and we price-match renewals, as well!
Walk-in renewals are also welcome Monday through Thursday between 10 am and 3:30 pm, and Friday between 10 am and 2:30 pm!*
Renewals by phone are available for qualifying patients.

Out-of-State Patients

Patients with a qualifying diagnosis and a valid driver's license from any US state may apply to the New Mexico Medical Cannabis Program to purchase and use medical cannabis in the state of New Mexico.
The process for out-of-state patients is currently the same as it is for New Mexico patients. Qualifying patients will need to be able to meet with a provider at Zia Health and Wellness in Albuquerque.

Grower's Applications

Patients who are interested in growing their own medical cannabis may apply to do so once accepted for enrollment in the New Mexico Medical Cannabis Program. Patients may pick up a free grower's application from our office anytime, or print one here: https://nmhealth.org/publication/view/form/136/.
A state's fee will apply.**

Caregiver Applications / Background Checks

We carry caregiver's applications and provide background check services for those who would like to apply to be a current patient's caregiver. Caregivers may purchase and transport medical cannabis for patients who are unable to do so themselves, and will need a certifying medical provider's approval. Patients may pick up a free caregiver's application from our office anytime, or print one here: https://nmhealth.org/publication/view/form/133/. 
A provider's sign-off fee and background-check fee will apply. 

Card Replacement Applications 

Patients who have misplaced their medical cannabis card may apply for a replacement card. Replacement applications may be picked up from our office for free, or may be printed here: https://nmhealth.org/publication/view/form/134/. Please note that Zia Health & Wellness is not responsible for lost, misplaced, or stolen applications. 
A state's replacement-card fee may apply.**

CBD Products and Merchandise

Cannabidiol, or CBD, is derived from medical cannabis, is non-psychoactive and aids with sleep, pain relief, and more. Patients do not need a medical cannabis card to purchase CBD products.

We carry a variety of CBD products such as tinctures, salve, hemp bud, lollipops and gummies. To purchase, visit our CBD link or stop by our office today for a look at what's in stock. We also carry pipes and screens, bongs, rolling paper, grinders, lighters, t-shirts, caps and more!

Discounts

We offer Veteran discounts and discounts for patients 65 years and older.

Handicapped Parking Permits

Eligible patients may qualify for a handicapped parking permit application. Give us a call or stop in during regular office hours for more information.
